HCC & SDOH Documentation & Coding

Jackie King will review the “ICD-10-CM Official Guidelines for Coding and Reporting” as they relate to the SDOH category of “Z-Codes.” These Social Determinants of Health reflect the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. We will discuss methods to support accurate documentation and coding capture for reporting on medical claims now and in the future.

To better prepare hospitals and providers for Value Based Reimbursement, Jackie King will also review the definition and premise behind the reporting of HCC ICD-10-CM codes to Medicare and other payers, including documentation and coding requirements, and how HCC codes affect beneficiary Risk Adjustment Scores.
